KEY JOB RESPONSIBILITIES: To provide supportive services to help meet the needs of the terminally ill Hospice patient and family as needed. To provide assistance and understanding to the family in time of bereavement. To work as a member of the Hospice team in providing Hospice care. Responsible for developing, implementing, and supervising the bereavement program and for the delivery of related services.

Additionally, the Bereavement Coordinator will:

• Manage the bereavement services program utilizing professional staff and volunteers.
• Oversee adequacy and appropriateness of bereavement programs for patients and family members.
• The clinical record shall contain notes for each service provided. A clinical note will be completed for each visit made to a patient’s home (including a private residence, assisted living facility, group homes, or skilled nursing facility) within 48 hours of the visit.
• Develop new bereavement programs and services as needed.
• Develop educational programs and materials for patients/families, staff, and the community regarding loss, grief, and coping with bereavement.
• Assist Hospice in educational training programs.
• Design materials for distribution to families eligible for and/or receiving Hospice bereavement services.
• Plan, implement, and supervise bereavement group events.
• Supervise support staff involved in the bereavement program.
• Oversee bereavement follow-up by patient care staff and volunteers.
• Oversee the volunteer component of the bereavement services.
• Conduct the bereavement section of the IDG conference.
• Participate in the maintenance of the bereavement component of the community and staff resource libraries.
• Participate in the orientation and training of new employees and volunteers working in the Hospice program.
• Prepare reports as requested by management.
• Attends IDG conferences.
• Works with the IDG concept of patient care.

Qualifications

PROFESSIONAL EXPERIENCE/QUALIFICATIONS:

• Maintains the agency’s mission, philosophy, and core values.
• Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety, and emergency management.
• Always maintains patient confidentiality, including all HIPAA regulations.

Regulatory Requirements

• Licensed or registered in the State of PA.
• Must pass a criminal background check & MVR check.
• Completed health statements acknowledging the ability to perform the duties of the position.
• Valid state driver’s license.
• Proof of current automobile insurance.
• Current CPR card; On-line CPR certification accepted; Hands-on CPR certification preferred.
• TB testing per agency policy; (1 or 2 step TB skin test within 12 months of hire & annual TB symptom screening thereafter).

Industry experience

• Experience in a Hospice setting preferred.
• Ability to work independently, make accurate, and at times, quick judgments. Ability to respond appropriately to crises outside of a hospital setting. Acceptance of and adaptability to different social, racial, cultural, and religious modes.
• Must be able to work independently, make judgments based on assessments and data available, and act accordingly. Must be flexible, innovative, and possess good interpersonal skills. Must be able to cope with mental and emotional stress and demonstrate emotional stability.
• Active patient contact within the past three years is preferred.
• Must have experience or education in grief or loss counseling.

EDUCATION:

• Graduate from an accredited institution of social work required.
