The Philadelphia Model for addressing Serious Mental Illness (SMI) in long term care provides highly structured therapeutic residential behavioral healthcare for frail adults who require skilled nursing level care and behavioral health services. Individuals served may have complex psychiatric needs and have reached maximum benefit from other community or hospital based mental health resources.

Overview
The Behavioral Wellness Center at Girard is seeking a licensed Social Worker to support adults receiving behavioral health and mental health services in a structured, onsite treatment environment. This role focuses on clinical social work, care coordination, and interdisciplinary collaboration to support individuals with serious mental illness.
Key Responsibilities
• Conduct comprehensive psychosocial assessments
• Participate in individualized treatment planning and care coordination
• Provide clinical social work services and ongoing case management
• Link clients to community resources including healthcare, benefits, housing, and social services
• Maintain accurate and timely documentation in the Electronic Health Record (EHR)
• Participate in interdisciplinary team meetings and clinical case reviews
• Support discharge planning and continuity of care
• Ensure compliance with HIPAA, confidentiality standards, and regulatory requirements
• Collaborate with internal and external providers to support client stability and recovery
Qualifications
Education and Licensure
• Master’s degree in social work (MSW) required
Experience and Skills
• Experience working with individuals with serious mental illness strongly preferred
• Experience in behavioral health, mental health, long term care, or human services settings
• Strong knowledge of community resources and social service systems
• Excellent clinical documentation and organizational skills
• Ability to manage a caseload in a fast paced, fully onsite environment
• Proficiency with Electronic Health Records (EHR)
