Job Summary
The Behavioral Health Patient Navigator provides support to individuals seeking behavioral health services by conducting initial screenings, coordinating referrals, and connecting clients with appropriate resources. This role works closely with providers, care teams, and community partners to ensure patients receive timely, compassionate, and coordinated care.
Essential Job Functions
- Conduct initial behavioral health screenings and preliminary diagnostic assessments.
- Coordinate internal and external referrals and provide ongoing case management support.
- Connect patients with community resources and higher levels of care when appropriate.
- Collaborate with providers, prescribers, and interdisciplinary care teams to coordinate treatment.
- Assist with care plans, consents, and collaborative care documentation.
- Maintain timely, accurate documentation in accordance with agency and regulatory standards.
- Monitor patient progress and communicate changes to the care team.
- Participate in staff meetings, care coordination activities, and CHS safety initiatives.
- Maintain professional boundaries, confidentiality, and ethical standards.
- Perform other duties as assigned.
Qualifications
Education
- Bachelor's degree in Human Services or a related field preferred.
Experience
- 2–3 years of experience in community mental health, case management, or a Federally Qualified Health Center preferred.
- Experience with integrated behavioral health models and diverse patient populations preferred.
Skills & Abilities
- Knowledge of behavioral health, community resources, and case management principles.
- Experience with crisis intervention, suicide risk assessment, and co-occurring mental health/substance use disorders preferred.
- Familiarity with electronic health records (EPIC preferred).
- Strong communication, interpersonal, and organizational skills.
- Bilingual (Spanish/English) preferred.

- Primarily a seated position requiring frequent computer use.
- Fast-paced healthcare environment with occasional exposure to emotionally challenging situations.
