Job Overview

Responsibilities
– Conduct comprehensive evaluations and assessments for patients.
– Develop and implement individualized treatment plans based on patient needs.
– Provide medication management and monitor patient responses to treatments.
– Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care.
– Maintain accurate and up-to-date patient records using electronic health record systems such as EPIC.
