Essential Responsibilities
- Conduct comprehensive psychosocial assessments of children, adolescents, and families.
- Develop individualized service plans that address behavioral health, educational, social, and family needs.
- Provide ongoing case management, counseling, crisis intervention, and emotional support.
- Assess risk factors related to child safety, abuse, neglect, domestic violence, and mental health concerns.
- Coordinate services with schools, healthcare providers, child welfare agencies, community organizations, and government agencies.
- Advocate for clients to ensure access to housing, healthcare, educational services, financial assistance, and behavioral health treatment.
- Facilitate referrals to community resources and monitor follow-up services.
- Conduct home visits, school visits, and community outreach as required.
- Maintain accurate, timely, and confidential documentation in accordance with agency, Medicaid, state, and regulatory standards.
- Participate in multidisciplinary case conferences and treatment planning meetings.
- Educate families on parenting skills, child development, coping strategies, and available community resources.
- Provide crisis intervention and safety planning during emergencies.
- Monitor client progress toward established goals and revise care plans as appropriate.
- Maintain compliance with HIPAA, agency policies, state regulations, and ethical standards of social work practice.
Qualifications
- Master's Degree in Social Work (MSW) required.
