Job Overview
We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong background in patient care, with experience across various clinical settings, including long-term care, assisted living, and acute care environments. As an LPN, you will play a vital role in delivering exceptional nursing services, supporting patient well-being, and ensuring compliance with healthcare standards.
Duties
- Provide direct patient care by administering medications, injections, and treatments in accordance with physician orders and established protocols.
- Conduct thorough patient assessments, monitor vital signs, and document clinical observations accurately within electronic health record (EHR) systems.
- Assist with diagnostic evaluations such as specimen collection and processing, and perform basic procedures including catheterization and sterile techniques.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans that address patients’ physical, emotional, and cognitive needs.
- Manage emergency situations effectively by utilizing first aid skills, airway management techniques, and critical care procedures when necessary.
- Ensure compliance with HIPAA regulations and maintain strict confidentiality of all patient information.
- Educate patients and their families on health management strategies, medication administration, and post-discharge care instructions.
Qualifications
- Valid state licensure as a Licensed Practical Nurse (LPN).
- Proven experience working in diverse healthcare settings such as long-term care facilities, hospitals, or assisted living communities.
- Familiarity with EMR/EHR systems such as Epic, Cerner, or Athenahealth for accurate medical documentation.
- Knowledge of medical terminology, anatomy, physiology, and nursing best practices including IV infusion and infection control protocols.
- Experience working with geriatric populations or individuals with developmental disabilities is highly desirable.
- Strong communication skills to effectively coordinate with patients, families, and healthcare team members.
- Ability to handle multiple priorities efficiently while maintaining attention to detail in high-pressure situations.
