We support over 600 individuals with nearly 40 locations throughout Rockland County.
POSITION SUMMARY:
The Nurse Practitioner (NP) will render primary care, medication management & coordination of care services. The NP works under a Collaborative Practice Agreement with the Medical Doctor.
POSITION QUALIFICATIONS:
Minimum Education: Certified and registered as a Nurse Practitioner by NYSED.
Minimum Experience: 2 years’ experience in an outpatient setting with the population served.
Required Certification/Registration: New York State Department of Education licensure and registration; NYS DEA registration for prescribing medication.
SPECIFIC JOB DUTIES AND RESPONSIBILITIES:
- Deliver quality health care through proper diagnosis and treatment of patients, utilizing evidence-based medicine and standards of care.
- Provide adequate and appropriate follow up, documentation and communication with patients, family members and support staff while complying with all State and Federal regulations.
- Assist with development of an organizational and administrative structure that assures adequacy and quality of care.
- Adhere to and enforce all State and Federal guidelines related to patient care and corporate compliance.
