Support youth as they prepare for adulthood through hands-on mentorship and group trainings, skill-building, and real-world experiences. The Arc of Washington County (est. 1952) is seeking a Transition Specialist to help youth build independence, explore career paths, and develop the skills needed for success in employment, education, and community life.
✨ What You’ll Do
- Provide personalized vocational, social skills/life skills, and real-world functional instruction to youth through hands-on individual mentorship and small group settings in the home and community.
- Support transition services in alignment with the Title I, Part D MOU, including coordinating school and community-based learning opportunities for the youth residential programs.
- Assist youth in developing career interests, workplace readiness, life skills encompassing the Ready by 21 Benchmarks (Financial Literacy, Civic Engagement, Well Being, Health Relationships, Housing, Education, and Employment) and post-secondary transition skills through job coaching, training, and supportive case management.
- Provide supervision, guidance, and direct support to youth in accordance with ISP/PCP plans, while supporting DSP youth staff.
- Collaborate with the Employment Transition Specialist as needed, while primarily supporting the Youth Programs.
- Participate in community-based activities and events, including some weekends and evenings.
- Complete initial RCYCP Certification meeting all requirements to work with youth within 180 days. Complete required Relias trainings (including Mandt) and maintain 40 hours of annual external trainings to support RCYCP certification process.
- Maintain knowledge of Children’s Program policies, person centered programming, safety protocols, and reporting requirements.
- Complete documentation accurately and on time (MARs, logs, incident reports, Care Tracker, attendance, Paycor, life skills tracking)
- Support youth participation in community and recreational activities and implement/track progress on treatment plans and goals.
- Maintain program environments, including Arc homes and vehicles, ensuring cleanliness and timely reporting of issues to appropriate supervisor.
- Attend staff meetings, complete assigned duties, and follow all reporting, call-in, and leave procedures.
- Maintain basic understanding of CARF standards and demonstrate professionalism, kindness, and respect in all interactions.
Requirements
- Must be at least 21 years of age
- Associate’s or Bachelor’s degree (AA or BA/BS) preferred, or equivalent coursework with 4 years of experience in workforce development, job coaching, or a related field
- Flexible, positive, and able to work independently; self-motivated with initiative
- Strong organizational and collaboration skills
- Valid U.S. driver’s license with at least 2 years of driving experience (excluding permit time) and a good driving record; able to travel to schools, residential programs, and community businesses
- Ability to implement positive behavior supports and safety practices
- Must pass background checks (state & national), fingerprinting, drug screen, and physical/ergonomic testing

- Part-time position (20 hours/week) within the Youth Program
- Opportunity to pick up additional hours as a DSP in residential programs, if desired, to maintain full-time designation.
