Transition Specialist – Full-Time | Vocational Services | Brick, NJ
Schedule: Full-Time (8am to 4pm) | Hourly | Daytime & community-based hours

Rate of Pay: $21.50 per Hour

As a Transition Specialist within our Vocational Program, you will play a key role in helping adults develop the skills, confidence, and real-world experience they need to move toward competitive, integrated employment.

What You’ll Do
As a Transition Specialist, you will support individuals through structured, goal-based activities designed to promote independence, employability, and community involvement:
Skill-Building & Participant Support
- Teach daily living skills, employability skills, social skills, and problem-solving strategies
- Implement structured activities and curriculum that support readiness for employment
- Adapt support based on each participant’s strengths, needs, and barriers
- Encourage positive social behavior, independence, and personal choice
Community-Based Learning
- Support participants at community job sampling sites, volunteer placements, and other integrated settings
- Build and maintain partnerships with local businesses and community organizations
- Transport participants to and from community-based sites using agency vehicles
Program Planning & Documentation
- Maintain knowledge of each participant’s Individual Service Plan (ISP)
- Review ISPs at least annually and recommend updates based on progress
- Assist individuals in identifying personal goals and strategies for achieving them
- Complete required documentation including daily logs, critical logs, medication records, and incident reports
- Implement behavior support plans and provide crisis management when necessary
Health, Safety & Compliance
- Administer medications according to agency standards
- Follow all treatment plans including dietary, behavioral, and adaptive equipment needs
- Ensure participant rights, confidentiality, and safety at all times

What We’re Looking For
- Bachelor’s degree in Human Services or related field (required)
- Valid NJ Driver’s License with no more than 5 points
- Strong verbal and written communication skills
- Ability to problem-solve, work independently, and collaborate with a team
- Comfortable networking and building relationships with community partners
- Ability to meet physical requirements (including lifting 25 lbs, assisting with mobility, and navigating community sites)
