Developmental Disability Waiver Support Coordinator

The Support Coordinator is a key member of our team, responsible for coordinating support services for individuals with developmental disabilities and receiving Medicaid Waiver active services. The Support Coordinator works closely with individuals, families, caregivers, and service providers to ensure that each individual supported receives the support they need to achieve their goals and live independently.

Key Responsibilities:

Case Management

  • Manage caseload and build relationships with individuals and service providers.
  • Conduct comprehensive assessments and develop/update Individual Service Plans (ISP) annually with input from individuals and their support team.

Service Coordination

  • Connect clients to community services as outlined in their Person-Centered Individualized Service Plan (ISP).
  • Support implementation of service plans through follow-ups and ongoing assessments, educating individuals on adherence.

Site Visits and Monitoring

  • Conduct face-to-face visits every 90 days (or 30 days for Enhanced Case Management) and monitor service quality through satisfaction evaluations.

Collaboration

  • Partner with public school systems to aid the transition of special education graduates to adult programs.

Documentation

  • Complete required documentation, including progress notes and service plans, ensuring timely EHR entry while meeting regulatory standards.

Community Integration

  • Collaborate with individuals to identify and access opportunities for community engagement.

Professional Development

  • Engage in annual training and professional development activities.

Other Duties:

  • Assess individual support needs and tailor service plans accordingly.
  • Research and connect individuals to local services for improved quality of life.
  • Coordinate logistics such as transportation and housing for service access.
  • Regularly evaluate support effectiveness and maintain accurate records.
  • Communicate updates with stakeholders and ensure confidentiality.
  • Collaborate with various professionals for comprehensive support and stay informed on best practices.
  • Perform other assigned responsibilities as necessary.

Education: Bachelor’s degree in a related field, such as social work, psychology, or disability studies.

Experience:

  • Minimum 1-2 years of experience working in a support coordination role or a related field.

  • Experience and skilled with Electronic Health Records (EHR) systems and/or skilled in various technology platforms.

Hours: The standard work week at The Arc of Northern Virginia is 37.5 hours per week. The hours for this position can be flexible, although they include some evening and weekend commitments based upon the availability of individuals and families, as well as the timing of organizational events.  Telework is offered and will be determined as appropriate by supervisory staff during and following a successful 90-day probational period. Some of the onboarding online trainings can be completed at home.

Minimum Qualifications:

Educational Background: Bachelor’s degree from an accredited four-year institution in a relevant human services field, such as Social Work, Psychology, Education (emphasizing psychology or special education), Human Development, Human Services, or Special Education.

Prior Experience: Minimum 1-2 years of experience working in a support coordination role or a related field.

Communication Skills: Must exhibit exceptional oral, written, and interpersonal communication skills. Strong critical thinking abilities are essential for advocating and navigating complex systems.

Organizational Skills: Strong organizational capabilities and proficiency in multitasking, ensuring efficient management of diverse responsibilities.

Case Management Knowledge: Solid understanding of case management services and the specific needs of adults with Intellectual and Developmental Disabilities (I/DD).

Caseload Management: Ability to manage and sustain an active caseload (21-25), requiring adaptability, prioritization of tasks, and effective time management strategies.

Work Under Pressure: Ability to perform well under pressure, establish priorities, and meet deadlines consistently. Attention to detail is crucial to guarantee the accuracy and completeness of all records and documentation.

Individualized Service Planning: Competence in utilizing information from assessments, evaluations, observations, and interviews to formulate individualized service plans tailored to each client’s needs and objectives.

Relationship Building: Proficient in establishing and sustaining positive working relationships with employees, individuals and families, and community partners.

Independent and Team Work: Ability to work independently as well as collaboratively within a team setting. Demonstrates initiative and the capacity to make informed decisions in emergency situations.

Technical Proficiency: Familiarity with Microsoft Office and database applications, with the ability to effectively utilize technology for documentation and communication purposes.

Preferred Qualifications: Bilingual in Spanish or other specialized languages. Knowledge of treatment modalities and intervention techniques, such as behavior management, independent living skills training, supportive counseling, family education, crisis intervention, discharge planning, and service coordination.  Experience with Electronic Health Records (EHR). Understanding Medicaid Waiver services and all Medicaid requirements, timelines, and documentation.

Special Position Requirements: The candidate chosen for this position will be required to complete a Child Protective Services Central Registry check and a Criminal Background check to the satisfaction of The Arc of Northern Virginia, as well as drug and TB screening.  A valid motor vehicle driver’s license is required, and the candidate will be required to use their own vehicle to and from meeting appointments, with mileage reimbursement provided by The Arc of Northern Virginia.
