Registered Nurse (RN) I/DD WAIVER AGENCY -MUST WORK WEEKENDS PREFFER AMAP CERTIFIED
NATURE OF WORK: This professional position functions independently as a member of The Arc’s interdisciplinary treatment team for those clients that require services in a home-based setting. The RN acts as a medical resource person for team members and specifically as Supervisor overseeing care providers such as Licensed Practical Nurses, AMAP's and Community Support Specialist. This position will uphold the agency’s mission and value by offering professional nursing services to our clients to insure optimum service planning and delivery.
MINIMUM QUALIFICATIONS:
- Must have at least two years of experience as a Registered Nurse.
- AMAP certification is preferred but will assist with certification for the right candidate.
- Position will education Direct Care staff and provide training for AMAP employees.
- Must be able to work DAY SHIFT THURSDAY THROUGH SUNDAY (night on call every three weeks)
- Ability to perform Essential Duties as outlined below.
- High School Graduate or equivalent.
- Associate Degree in Nursing or Diploma.
- Current W.V. License from the West Virginia Board of Examiners for Registered Nurses.
- Previous experience working with individuals with a diagnosis in at least one of the following populations: MH, I/DD, or CD.
- Knowledge of nursing protocol, theories, principals, and practices of nursing.
- Ability to comply with Client’s Rights.
- Ability to comply with The Arc’s and Division departmental safety procedures.
- Ability to read, write, understand and speak the English language.
- Ability to legally transport clients.
- Must be currently practicing as an actively licensed registered professional nurse in good standing in West Virginia
- Must have practiced as a registered professional nurse in a position or capacity requiring knowledge of medications and health maintenance tasks in the immediate two years prior to being appointed the authorizing RN trainer/instructor for AMAP’s.
- Must be familiar with the nursing care needs of residents in the type of facility in which the unlicensed personnel will be administering medication and/or performing health maintenance tasks.
- Must be knowledgeable of State Regulations pertaining to the medication administration and performance of health maintenance task.
WORK ENVIRONMENT: In-Doors 90%, Out-Doors 10%
WORK PACE: Self-paced, requiring the ability to make decisions quickly and accurately in an often-times fast-paced, demanding work environment.
ESSENTIAL DUTIES:
- Oversees the personal care (i.e. grooming, bathing, dressing, feeding) of clients in a Group Home setting.
- Records or directs to have recorded client’s vital signs, weights, measurements, intake and output, food consumption and other information as ordered.
- Performs or directs to perform range of motion and reality orientation with clients.
- Administers or directs to have administered all medications as order by a physician.
- Supervise care providers such as Licensed Practical Nurses, Certified Nursing Assistants and Health Care Assistants through appropriate training. Upon Certification, this will include AMAP oversight.
- Complies and implements or directs to have complied and implemented all physicians’ orders.
- Helps to develop, reviews and ensures implementation of care plans in compliance with The Arc’s and State guidelines and physician’s orders.
- Ensures the timeliness and accuracy of information documented into the client’s records.
- Along with the RN Supervisor, assures that monthly and/or quarterly physician’s orders are accurate and completed.
- Follows up on new physician orders for clients.
- Once trained will provide monthly Clinical Supervision with LPN’s.
- Attends IDT Meetings.
- Ensures that client medications are available within the Group Homes.
- Participate as a member of the professional interdisciplinary treatment team as client needs dictate.
- Conducts trainings as assigned.
- Completes documentation as required by The Arc, Medicaid, OSHA, and other licensing agencies. Employee ensures that documentation (such as Progress Notes, SAL’s, DD Forms, etc.) are completed accurately and timely and will reflect services the client(s) has received as appropriate.
- Follows approved standard nursing practices and guidelines when administering or directing to have administered all medications to clients. This will include proper documentation and timeliness of the administration of medications as well as observance of the client. Administer medications to patients as prescribed by their physician/
- Along with the RN Supervisor, does monthly medication reconciliation logs to assure that medications are not expired and if they are, follows proper protocol to ensure they are disposed over properly and in a timely manner.
- Performs personal care nursing assessment for community clients when directed.
- Completes in a timely manner the monthly or quarterly nursing assessment per the client’s treatment plan if the IDT requires it.
- As approved, will serve as AMAP RN and provides training, oversight and compliance in all areas for this.
- Adhere to professional code of ethics in performing nursing services.
- Other duties as assigned.
PHYSICAL DEMANDS:
May require extended workdays of up to ten to twelve hours per day.
Sitting: 2 Hours per 8 hour work shift. Alternates frequently to walking and standing.
Standing: 4 Hours in an 8 hour work shift. Alternates frequently to walking and occasionally sitting.
