Registered Nurse (RN) – Adult Family Care (AFC)
Adult Family Care (AFC) Program:
Pay: $38.00–$41.00 per hour
Schedule: Full-time, 40 hours per week, Monday–Friday, 9am to 5pm, no nights or weekends
Work Location: In person – Haverhill, MA
About The Arc
The Arc of Greater Haverhill-Newburyport is a grassroots 501(c)(3) nonprofit founded in 1962 to advocate with and for individuals with intellectual and developmental disabilities and their families throughout the Merrimack Valley and Seacoast communities.
We are currently seeking a full-time Registered Nurse (RN) to join our Adult Family Care (AFC) program. AFC is a MassHealth funded program that offers support to families that care for people with disabilities in a home setting. Qualified caregivers provide support to prevent or delay out of home placements for individuals that cannot live safely on their own.
This is a hands-off nursing position focused on assessment, care coordination and clinical oversight. The RN works closely with AFC Coordinators as part of a multidisciplinary team, providing nursing oversight, resource and education, and case management through home visits. By customizing services, The Arc optimizes the development and self-discovery of the whole person, their family, and how they relate to their community.
RN Key Responsibilities
- Conduct initial and ongoing assessments of clients' health status and safety in the community; identify potential resources needed; evaluate and maintain program eligibility; and complete bi-annual health assessments.
- Collaborate with the multidisciplinary team to develop and implement care plans with a focus on health maintenance and preventative care.
- Train, support, and supervise caregivers in partnership with the Care Coordinator.
- Provide oversight of medical needs, follow-up care, and communication with healthcare providers.
- Conduct monthly/bimonthly home visits and complete all required documentation.
Qualifications
- Valid Massachusetts RN License required.
- 2 years of recent experience in the direct care of elders or adults with disabilities required.
- Prior community-based nursing experience preferred.
- Experience with MDS nursing assessment tool preferred.
Fluent in Spanish highly desirable.
- Excellent written and communication skills.
- Valid driver's license and reliable transportation.
- Satisfactory background check and driving record.
- Computer skills, including experience with Microsoft Office, are required.
