Occupational Therapist

Description

SUMMARY: Plans, organizes, and conducts occupational therapy program to facilitate development and rehabilitation of mentally and/or physically disabled persons by performing the following duties.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

• Reviews Physician's referral and individual's condition and medical records to determine occupational therapy treatment required.
• Evaluates individual's abilities in order to establish an occupational therapy program which meets the individual's needs using formal assessment tools.
• Writes occupational therapy treatment plans based on assessment.
• Complete any forms for participation in insurance including, but not limited to Medicare and New York State Medicaid. 
• Selects therapy suited to individual's physical capacity, intelligence level, and interest to upgrade individual to maximum independence.
• Provides therapy in functional, prevocational, vocational, homemaking skills, and activities of daily living.

Requirements

EDUCATION and/or EXPERIENCE: New York State Occupational Therapy License.
