Location: Madison County
Schedule: Full-time with day, evening and weekend shifts available
Salary: $20.00
DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Provides individualized care as outlined in the individuals Life Plan, or ISP.
- Assists Individuals in the primary activities of daily living (toileting, feeding, cooking, cleaning) as outlined in individual's ISP to promote independence.
- Maintains files and produces service notes and documentation on assigned caseload.
- Documents and submits reports to the program as outlined in the assigned department.
- Model appropriate social skills to assist the individual in developing and maintaining relationships in the home and in the community
- Assists Individuals in the development of natural supports within the community.
- Attends all established or assigned in-services and web-based training.
- Provides First Aid, CPR, and SCIP-R as needed.
- Uses behavior management techniques as directed in the individual behavior plans and/or medication monitoring plans.
- Give medications as prescribed by physician, per OPWDD regulations
Requirements
- High school diploma or GED; Valid NYS driver's license and 12 months driving history. Six months to one-year related experience and or training preferred.
