Youth Community Enrichment Case Manager
Do you have a passion for helping children with intellectual and developmental disabilities (IDD) reach their full potential? The Arc Fond du Lac is seeking a compassionate and organized Youth Community Enrichment Case Manager to support youth participants and their families through our Youth Community Enrichment Program (YCEP).
Key Responsibilities
- Manage a caseload of youth with IDD and develop person-centered service plans.
- Coordinate participant admissions, assessments, annual reviews, and discharges.
- Work closely with families, guardians, schools, and service providers.
- Plan engaging activities, community outings, and skill-building opportunities.
- Ensure compliance with Wisconsin CLTS Waiver regulations and documentation requirements.
- Assist with staff training, onboarding, and program oversight.
- Support participant health, safety, and overall quality of care.
- Drive accessible vehicles for community-based activities as needed.
Qualifications
- Age 21 or older.
- Coursework in Advanced Early Childhood Development or at least 2 years of experience working directly with individuals with intellectual and developmental disabilities.
- Minimum 1 year of supervisory experience.
- Valid Wisconsin driver's license with a clean driving record.
- Strong communication, organization, and leadership skills.
- Proficiency with Microsoft Office.
- Ability to pass a background check and pre-employment drug screen.
ScheduleBenefits
- Full-time, exempt position
- Primarily weekday hours
- Required 1 Saturday per month during the school year
- Occasional evening/weekend hours as needed
