The Department

HARP (Hospital Admission Risk Program) Complex Care is largely a community-based service within the Health Independence Program (HIP). HARP sits within the Alfred Home Acute and Community (HAC) Program and reports through the Acute and Integrated Stream (AIC). HARP provides education and care coordination to improve the health literacy, well-being, and health independence of individuals living in the community. The service promotes patient flow and aims to reduce potentially preventable hospital readmissions.

The Role

This position is open to experienced registered nurses with a strong interest in chronic and complex holistic health care.

HARP complex care: The HARP Complex Care Clinician is part of a geographic team, providing supervision and support to the multidisciplinary team, working alongside the nurses, case managers, social workers, and allied health clinicians. The role is responsible for conducting phone risk assessments, triaging and allocating new referrals, and prioritising care. The HARP Complex Care Coordinator also conducts comprehensive and holistic systems' assessments, initiates interventions to improve patients' health independence, and provides support and education on the self-management of chronic disease. The HARP Complex Care Coordinator works in collaboration with the patient, family, GPs, the HARP medical registrars, carers and other community based services or hospital based resources. The role works in partnership with the multidisciplinary team and the HARP registrar in the holistic care of patients. The role has leadership responsibilities, supporting the HARP team leads and undertaking a quality improvement portfolio in addition to clinical work. On occassion, the role steps up to provide team leadership.

Essential

Qualifications and Experience Required

  • Four or more years of Nursing or Allied Health experience following graduating from a Bachelor of Nursing or relevant Allied Health degree
  • Excellent understanding of patient triage and clinical and psychosocial risk
  • Sound assessment skills, critical thinking and the ability to work autonomously within role scope and as part of a team.
  • Skills in improving patients' health states and quality of life through the promotion of chronic disease self-management and health independence
  • Ability to rapidly recognise deterioration and or risk in the community and to escalate to the appropriate clinical and managerial line of management
  • A strong interest in social equity and in caring for people who experience significant challenges in both chronic disease, vulnerability, and psychosocial complexities
