📍 Full-Time | Monday–Friday Rotating Weekends
💲 $45–$53/Hour

Are you a healthcare professional who thrives on care coordination, patient advocacy, and helping individuals achieve successful outcomes? We're seeking a compassionate and driven Case Manager to join our inpatient rehabilitation team and play a key role in guiding patients through their recovery journey.
In this role, you'll collaborate with an interdisciplinary team to ensure patients receive the support, resources, and care they need from admission through discharge.
What You'll Do
- Coordinate patient care throughout the rehabilitation process
- Develop individualized care plans alongside physicians, nurses, therapists, and families
- Lead interdisciplinary team meetings and treatment discussions
- Manage discharge planning and connect patients with appropriate community resources
- Educate patients and families on recovery goals and expectations
- Monitor patient progress, insurance requirements, and care outcomes
- Maintain accurate documentation and ensure regulatory compliance
- Support utilization review and quality improvement initiatives
What We're Looking For
✔ Active RN, Social Work, PT, OT, ST, Rehabilitation Counseling, or other qualifying healthcare license
✔ Associate Degree in Nursing required for RNs (BSN preferred)
✔ Bachelor's degree required for other eligible disciplines
✔ Rehabilitation experience preferred
✔ Ability to obtain CCM or ACM certification within two years of hire
