- Collaborate with interdisciplinary teams to coordinate patient care across various settings, including clinics and telehealth services.
- Monitor patient progress and adjust treatment plans as necessary, utilizing medical terminology effectively.
- Perform procedures such as tube feeding and sonography when required.

Qualifications
- Master’s degree in Nursing with a focus on Family Nurse Practitioner program.
- Current certification as a Family Nurse Practitioner (FNP) from an accredited board.
- Strong knowledge of geriatrics, memory care, and related healthcare practices.
- Familiarity with telehealth technologies and their application in patient care.
- Proficient understanding of medical terminology and infection control measures.
- Experience working in clinical settings is preferred; familiarity with Cerner or similar systems is a plus.
- Excellent communication skills, both verbal and written, with the ability to educate patients effectively.
