- Contribute as a core member of an interdisciplinary primary care team, collaborating with all members of the care team to co-manage chronic conditions;
- Provide primary care consultation, evaluating patients for mental health conditions, barriers to care and cognitive impairments;
- Develop and implement evidence-based treatment plans utilizing modalities like Cognitive Behavioral Therapy and Acceptance and Commitment Therapy;
- Educate patients and their support system on lifestyle modifications, behavioral health strategies and medication adherence; and
- Participate in the education of learners.
- Doctoral Degree in related field.
- Active Texas License with Texas Behavioral Health Executive Council.
