We strive for excellence through continuous growth, thoughtful decision making, and mission driven collaboration, supporting one another while embracing challenges that lead to meaningful, long-term impact.Job DescriptionPerforms moderately complex (journey-level) to advanced (senior-level) case management work with a primary focus of acting as a liaison between TJJD and local juvenile probation departments providing post-commitment services to youth pending admission to a TJJD operated facility and helping ensure probation departments have the information and resources needed to provide appropriate services. Following commitment of the youth by a juvenile court, TJJD facilitates the intake, monitoring and transition of youth pending admission into one of the TJJD operated facilities which requires collaboration between TJJD and local juvenile probation department to facilitate the exchange of information about the youths case and scheduling of an intake date. Youth remaining in pre-adjudication facilities, pending admission to TJJD, may need services provided by juvenile probation departments, in collaboration with TJJD, to begin rehabilitation and may include psychological and criminogenic risk and needs assessments, intake screenings, medical services, mental health crisis intervention, pre-treatment behavior monitoring, education services, and general rehabilitative concepts foundational to treatment completion in a TJJD operated facility. Work involves reviewing case files; participating in the intake and orientation of youth; collaborating in the development of individual case plans (including goal development) and reviewing monthly progress reports with facility staff, youth, and their family members or guardians; evaluating outcomes and providing technical assistance, guidance, and training as needed; coordinating medical treatment, and providing needed referrals; verifying services provided for grant disbursements; and coordinating intake, admission and transition planning with youth, their family members or guardians, facility staff, community providers, and parole officers to ensure a continuity of service delivery throughout the youths intake and admission to TJJD. Works with service providers to understand and deliver the tenants of The Texas Model.May be required to work in excess of 40 hours per workweek, different shift patterns, and be on-call. Regular travel may be required based on location of program assignment.Career Path Promotion Opportunity: The TJJD Admission Case Management Specialist job series has been established within the TJJD Case Manager career path. An employee is hired at a level corresponding with the career path education and experience requirements and may promote up to Case Manager IV without competing for a vacant position after the employee meets: (1) the education and experience requirements for the next higher level; and (2) the established performance criteria per agency policy (PRS.15.07).Pre-Service Training Academy Requirement: Newly hired case managers or rehired case managers who separated more than 18 months will be required to attend the pre-service training academy. Case managers who have worked for TJJD and return to employment within 18 months of their separation date are not required to attend the pre-service ac demy and will be placed in full-time status on their date of employment.Levels of Work Performed by Career Path Level:Case Manager II: Performs moderately complex (journey-level) work under general supervision with moderate latitude for the use of initiative and independent judgment.Case Manager III: Performs highly complex (senior-level) work under limited supervision with considerable latitude for the use of initiative and independent judgment.Case Manager IV: Performs advanced and/or supervisory (senior-level) work under minimal supervision with extensive latitude for the use of initiative and independent judgment.Essential Job DutiesReviews the youths master file and ensures all orientation and assessment documentation is complete; requests missing information; and shares appropriate information discovered during master file reviews with facility staff; collaborates in the development of rehabilitation goals for case planning; ensures completion of appropriate intake assessments and screenings; and facilitates continuity of care during transition of youth into TJJD operated facilities. Conducts ongoing monitoring of services and the youths behavior; verifies the provision of services by the local juvenile probation department, youth attendance and engagement in provided services; coordinates required documentation for youth admission and movement into a TJJD operated facility; facilitate quality collaboration; verifies accuracy of grant disbursement reports; assists with preparing materials in response to requests for grant program information; address needs as they emerge; and ensures continuity of services as youth transition into a TJJD operated facility.
