Clinical Assessment and Care Planning,Care Coordination and Systems Navigation,Client Advocacy,Family Support and Communication, Psychosocial and Quality of Life Support, Home Safety and Functional Support,Medical Health and Monitoring, Caregiver and Companion Oversight, Documentation and Professional Reporting, Resource Identification and Benefits Navigation, Crisis Management and Risk Mitigation, End of Life and Palliative Support, Community and Lifestyle Integration.
