The Speech Language Pathologist, or Speech Therapist, is responsible for direct patient care for hospital inpatients and outpatients. SLP will abide by all responsibilities as outlined by their contract with TVHC.


Job Description:

The SLP examines each individual, develops a Plan of Care (POC), and continually reassesses the patient's status and adjusts the therapy techniques needed. The SLP is also responsible for consulting with the patient or responsible person, the patient's medical provider and other agency personnel regarding the POC. The SLP services are provided in accordance with provider's orders and as defined by the SLP standards of practice. The SLP performs all duties in a professional and ethical manner, collaborating with the other members of the health care team, ensuring quality patient care.

Clinical Care

The staff SLP is required to provide speech language pathology services in inpatient, acute and swing bed, outpatient and outpatient in the home.

The staff SLP is required to see all types and ages of patients including pediatric and geriatric.

Evaluation and treatment areas include speech, language, voice, fluency, cognition, and swallowing. Evaluations for swallowing may include bedside evaluations and modified barium swallow studies.

The SLP will review the SLP referral, patient's medical records, and patient's goals to determine appropriate SLP treatment plan.

The SLP may confer with physician, mid-level providers and other practitioners to obtain additional patient information, suggest revisions in treatment program, and integrate speech language pathology treatment with other aspects of patient's health care.

Provides written evaluation and treatment plan.

Provides a written plan of care to be signed by the provider.

Updates plan of care as needed.

Evaluates effects of treatment at various stages and adjusts treatments to achieve maximum benefit.

May perform in home evaluations of the patients’ living situation and provide recommendations for inpatients, swing bed patients and outpatients when appropriate.

Reviews other diagnostic testing i.e. lab and radiology to help develop or revise treatment programs.

Instructs, motivates, and assists patients to perform various activities.

Records treatment, response, and progress in patient's EMR.

Enters billing for services provided in the EMR

Instruct patient and family in treatment procedures to be continued at home, and environmental adaptations including equipment and/or designing adaptations to improve function

Provides patient and family education

Uses creative media as treatment method for cognition and language skills

Attend meetings as requested by Administration or other hospital staff

Comply with all compliance and regulatory initiatives of the hospital

Participates in rehab departments performance initiative projects as outlined by the rehabilitation director
