Responsibilities
- Provide personalized assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patients’ vital signs and observe for any changes in health or behavior, documenting findings accurately and promptly
- Assist with medication administration following HIPAA guidelines and care plans established by healthcare professionals
- Support patients during meal preparation, feeding, and ensure proper nutrition tailored to individual needs
- Utilize specialized equipment such as Hoyer lifts or resident’s
to safely transfer individuals with mobility challenges - Maintain a clean and safe environment through regular cleaning duties aligned with health standards and infection control protocols
- Collaborate with nursing staff, social workers, and other care team members to develop and implement effective care plans
Skills
- Proven experience in group home settings, assisted living facilities, or home & community care environments
- Knowledge of patient observation techniques, vital signs measurement, and basic nursing skills including CPR and first aid certification
- Familiarity with dementia care, Alzheimer’s care, memory care, hospice care, and working with individuals with developmental disabilities or autism spectrum disorder
- Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts
- Excellent communication skills for working effectively with patients, families, and interdisciplinary teams
- Strong understanding of HIPAA regulations to ensure patient confidentiality and data security
- Experience in medication administration, care plan adherence, and working within long-term care or nursing home settings
