Role Description The Case Manager is a full-time, on-site role located in Houma, LA. This position is responsible for coordinating comprehensive care plans for patients receiving home health services, ensuring that medical, social, and support needs are met. Day-to-day tasks include assessing patient needs, developing and updating care plans, monitoring progress, and collaborating with physicians, nurses, therapists, and family members. The Case Manager also facilitates referrals to community resources, manages documentation and compliance with regulatory requirements, and advocates for patient needs to promote quality, safety, and continuity of care. The role involves regular in-person communication with patients, families, and the clinical team, as well as timely reporting and follow-up on changes in patient status.
