What are the responsibilities and job description for the Care Coordinator position at Tennyson Center For Children?
Salary Range: $49,800 - $55,102.60
The Care Coordinator is responsible for the administrative duties of processing and screening of all inquiry and referrals for Tennyson programs. This includes completing clinical screenings of potential clients and making recommendations for appropriate services. When Tennyson is not an appropriate match for services, the Administrative Care Coordinator will work to refer clients to other, more appropriate programs. They are responsible for understanding HIPAA guidelines and following them to ensure compliance for all admitting clients.
Essential Job Functions
- Review and respond to all referrals within agency deadlines
- Engage in timely communication with referral sources and families
- Complete clinical screenings and assessments to determine appropriateness for programming
- Make recommendations for the services needed for a client and how Tennyson programs might be able to meet a client’s needs, including the necessary level of care
- Enter and track all referral information in a timely manner
- Cross functional collaboration within Tennyson’s teams for placements and recommendations of new referrals
- Works with a variety of insurances and payer sources to obtain contacts and payment information
- Schedules intake appointments based on clinician availability
- Administrative tasks such as emailing, calling, collecting documentation and more as needed
Other Job Functions
- Complete documentation of all contacts with clients in a timely manner based on agency standards
- Attend external clinical meetings for pending clients to discuss Tennyson services as requested by referral sources
- Connects referrals/clients to community resources as needed
- Reviews legal documents to determine custody arrangements of referrals to ensure the inclusion of the appropriate individuals in the intake process
Physical Requirements
- Minimal physical requirements. Ability to life office equipment up to 5 pounds as needed. Ability to engage in phone conversations and complete tasks on a computer.
Working Conditions
- Position may be remote up to 75% of the time
- Work will be split between a general office environment and remotely via telehealth; some work within the community may be needed
- Minimal travel throughout the Denver Metro area may be needed
Minimum Qualifications
- B.A. in Human Services or related field
- 2 years’ experience working with children/families who have experienced trauma
- Strong theoretical understanding of clinical issues, family and community systems
- Bilingual preferred
- All positions require criminal CBI background check, FBI fingerprint check, State of CO child abuse check, DMV driving record check, pre-employment drug test, pre-employment physical, academic credential verification, and reference check.
