Are you an organized, compassionate nurse who enjoys guiding patients through their healthcare journey? Tennessee Valley Urology Center is seeking an experienced Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our growing team as a Patient Navigator.

This role is ideal for a nurse who excels at communication, coordination, and patient advocacy. As a Patient Navigator, you will serve as the primary point of contact for patients, helping them understand their treatment plans, coordinating care, and ensuring they receive exceptional service from diagnosis through follow-up.

Responsibilities

  • Serve as the primary clinical resource for assigned patients.
  • Coordinate patient care throughout the treatment process.
  • Educate patients regarding diagnoses, procedures, medications, and follow-up care.
  • Review laboratory and imaging results with providers and communicate appropriate information to patients.
  • Triage patient phone calls and portal messages using established clinical protocols.
  • Coordinate referrals, diagnostic testing, and specialty services.
  • Collaborate closely with physicians, nurse practitioners, medical assistants, surgery schedulers, and ancillary staff.
  • Ensure timely documentation within the electronic medical record.
  • Assist with care coordination, quality initiatives, and patient satisfaction efforts.
  • Advocate for patients by helping remove barriers to care and improving the overall patient experience.
  • Maintain compliance with HIPAA and all clinical standards.

Qualifications

Required

  • Current Tennessee RN or LPN license in good standing.
  • Minimum of two years of clinical nursing experience.
  • Excellent communication and interpersonal skills.
  • Strong organizational skills with the ability to prioritize multiple tasks.
  • Proficiency with electronic medical records and basic computer applications.
  • Commitment to providing compassionate, patient-centered care.

Preferred

  • Experience in a specialty physician practice.
  • Urology, surgery, oncology, or care coordination experience.
  • Previous patient navigation or case management experience.
