What Youll Do
- Provide physical therapy services according to a physicians plan of care and state practice guidelines
- Evaluate patient function and assist in developing individualized care plans
- Educate patients, families, and caregivers on treatment plans and therapeutic activities
- Translate prescribed exercises into functional activities and activities of daily living
- Recommend modifications to equipment and home environments to support safety and independence
- Collaborate with physicians and interdisciplinary team members to coordinate care and monitor patient progress
- Document clinical notes, progress reports, and summaries in a timely and accurate manner
- Support and motivate patients and families throughout the rehabilitation process
- Supervise Physical Therapist Assistants in accordance with company policy and state regulations
- Maintain accurate clinical records and track patient goals and outcomes
Qualifications
- Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA)
- Current Physical Therapist (PT) license in the state of practice
- At least two (2) years of experience in an acute care or similar setting (preferred)
- Current CPR and First Aid certification
- Reliable transportation, valid drivers license, and current automobile insurance
