What Youll Do
As a Home Health LPN, youll provide hands-on, compassionate care while working closely with an interdisciplinary team:
- Deliver direct patient care in accordance with the state Nurse Practice Act
- Carry out care plans developed by the Registered Nurse
- Educate patients and families on self-care and treatment plans
- Document care accurately and timely
- Coordinate with RN and care team to ensure patients receive comprehensive support
- Utilize proper techniques for equipment, supplies, and infection control
- Support patients physical, emotional, and educational needs in the home
What Youll Need
- Active LPN/LVN license in the state
- Graduate of an accredited practical/vocational nursing program
- 3 years of nursing experience (Home Health, Med-Surg, or Community Health preferred)
- Current CPR certification
- Reliable transportation, valid drivers license, and auto insurance
