What You’ll Do
- Evaluate patients and develop individualized treatment plans based on physician orders.
- Provide skilled physical therapy services in the patient’s home.
- Help patients improve mobility, strength, balance, and overall functional independence.
- Educate patients, families, and caregivers on treatment plans, home exercises, and safety strategies.
- Recommend adaptive equipment and home modifications to promote safety and independence.
- Collaborate with physicians and the interdisciplinary care team to coordinate patient care.
- Accurately document evaluations, progress notes, and patient outcomes in a timely manner.
- Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations and company policy.
- Encourage and support patients throughout their rehabilitation journey.
Qualifications
- Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA).
- Current Physical Therapist (PT) license in the state of practice.
- Home Health experience preferred, but not required.
- Current CPR certification.
- Valid driver’s license, reliable transportation, and current automobile insurance.
