POSITION OVERVIEW:
The Psychiatric RN provides specialized behavioral health nursing care to patients in their homes. In this role, you’ll perform comprehensive assessments, monitor mental and physical health status, and coordinate personalized care plans as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. A few position highlights include:
- Conduct initial and ongoing nursing evaluations, assessing both behavioral health and medical needs, and identifying changes in patient condition.
- Provide patient and caregiver education on medications, symptom management, safety interventions, and coping strategies.
- Administer medications and perform lab draws and specimen collections per physician orders.
- Collaborate closely with the care team, including the clinical manager, psychiatrist, and other providers, to ensure effective and coordinated care.
- Maintain and submit accurate documentation and clinical notes, including the patient’s condition, appropriate goals, interventions, and patient’s response to home health care.
- Establish and maintain an effective and efficient visit route.
- Provide high quality clinical services within the scope of practice as defined by the state laws governing the practice of nursing, in accordance with the plan of care, from admit through discharge.
THE MUST HAVES:
- Current RN license in the state where services are rendered.
- Must have one of the following to meet CMS requirements for psychiatric nursing in the home care setting:
o An RN with a Master's Degree in a mental health nursing or psychiatric specialty. Must have nursing experience in an acute treatment unit or other psychiatric setting.
o An RN with a Bachelor's Degree in nursing. Must have one (1) year of recent nursing experience in an acute treatment unit or other psychiatric setting.
o An RN with a Diploma or Associate 's degree in nursing. Must have two (2) years of recent nursing experience in an acute treatment unit or other psychiatric setting.
o Please note: Experience must have been within the last three (3) to five (5) years. If not, documentation must support psychiatric retraining, classes, or CEUs to update psychiatric knowledge.
- Current CPR certification, driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
- Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.
