Responsibilities Include:
- Follow individualized care plans and document patient progress.
- Attend school with patient during the week
- Provide direct care to patients, administer medications, monitor vital signs, perform physical assessments and treatments.
- Communicate effectively with patients, families, and the healthcare team.
- Provide compassionate care in the patient's home, fostering a supportive, respectful environment that prioritizes their well-being.
- Work independently and manage patient care in a home setting
- Ability to think critically and make decisions when caring for patients in their home environment.
- Perform physical tasks, including lifting and mobility assistance.
- Complete electronic documentation and paper documentation that maintains compliance with all state and federal guidelines.
- Participate in continuing education as required.
Required Qualifications Include:
- Active RN/LPN license in good standing
- CPR certification
- Familiarity with HIPAA.
- Candidates must be able to lift 50 pounds and pass a fit-to-work assessment
Other:
- Weekends and Holidays are NOT required but available to those seeking other shifts
