Tendercare Home Health is seeking dedicated and compassionate Private Duty RNs & LPNs to join our growing team, serving Indianapolis and the surrounding areas. In this role, you'll have the opportunity to make a meaningful impact from day one. We're looking for someone who has experience with vents, trachs and gtubes for multiple pediatric cases.

Responsibilities Include:
- Follow individualized care plans and document patient progress.
- Ability to use critical thinking to assess, plan, and implement care in a home environment.
- Recognize and report changes in client’s condition to the Case Manager and Physician in a timely manner.
- Provide direct patient care, including but not limited to, medication administration and vital sign monitoring, physical assessments and treatments.
- Accurately complete both electronic and paper documentation in compliance with state and federal guidelines.
- Promote personal safety and maintain a safe environment for clients, including adherence to infection control protocols.
- Communicate effectively and professionally with patients, families, and members of the healthcare team.
- Perform physical tasks, such as lifting, repositioning, and providing mobility assistance to ensure patient comfort and safety.
- Work independently to deliver compassionate patient-centered care that promotes dignity and emotional well-being in a respectful home environment.
- Participate in continuing education as required.
Required Qualifications Include:
- LPN OR RN Graduate of an accredited School of Nursing
- Active Indiana LPN OR RN license in good standing
- Current CPR certification
- Strong written and verbal communication skills
- Knowledge of HIPAA.
- Candidate must be able to lift 50 pounds and pass a fit-to-work screen

- Days
- Nights
- Weekends
- Part Time
- Full Time
- Weekends and Holidays are NOT required but available to those wanting other shifts
