Pay Range: $23.00 – $25.00, depending on related experience and education
Schedule: Full-time | Monday-Friday 8:00am - 4:30pm
The Role
At TLC of Duluth, we help people with disabilities build practical skills, make meaningful choices, and stay connected to their community. We are looking for a Client Care Manager who can pair strong organization with genuine relationship-building. This full-time role coordinates services for an assigned group of clients, guides direct support staff, develops person-centered plans and turns them into consistent day-to-day support. You will be part of a small, locally-owned business where clear communication, sound judgment, and follow-through make a visible difference.
What You Will Do
- Serve as a Designated Coordinator under Minnesota Statutes, section 245D.081, and coordinate the delivery and evaluation of services for assigned clients.
- Oversee TLC's responsibilities in each client's support plan and support plan addendum, and take action to help the client make progress toward identified outcomes.
- Translate person-centered goals into practical service strategies, measurable outcomes, and useful data systems that show whether supports are working.
- Instruct, coach, and assist direct support staff in implementing support plans and service outcomes; review service delivery to assess and document competency.
- Evaluate the effectiveness of services, support methods, and progress using measurable and observable criteria; recommend adjustments when needed.
- Act as the primary TLC contact for assigned clients and communicate professionally with clients, families, legal representatives, case managers, support teams, and community partners.
- Complete accurate, timely service documentation, including intake and annual documents, progress reports, meeting notes, support-plan materials, incident reports and internal reviews.
- Protect each client's health, safety, rights, dignity, confidentiality, and professional boundaries while promoting choice, independence, and community participation.
- Support staff orientation, training, performance feedback, annual evaluations, and competency verification in partnership with other TLC leadership.
- Help prepare for state licensing reviews or investigations; maintain a working knowledge of applicable 245D requirements, TLC policies, and assigned compliance responsibilities.
- Provide direct client support when needed and participate in shared on-call or emergency-response coverage as scheduled.
Minimum Qualifications:
To meet Minnesota 245D Designated Coordinator requirements, you must qualify through one of the following paths:
1. (PREFERRED) A bachelor's degree in a field related to human services, education, or health, plus one year of full-time qualifying work experience;
2. An associate degree in a field related to human services, education, or health, plus two years of full-time qualifying work experience;
3. A diploma in a field related to human services, education, or health from an accredited postsecondary institution, plus three years of full-time qualifying work experience; or
4. At least 50 hours of education and training related to human services and disabilities, plus four years of full-time qualifying work experience under the supervision of a staff person who meets one of the degree or diploma pathways above.
Qualifying experience includes providing direct care to people with disabilities or people age 65 and older, or equivalent work providing care or education to vulnerable adults or children. TLC will verify education, training, experience, and role-specific competence.
You will also need:
- The ability to develop effective person-centered service plans and design or use data systems to measure the effectiveness of services and supports.
- Strong written and verbal communication skills, professional judgment, reliability, and the ability to give and receive clear direction and constructive feedback.
- Ability to readily navigate local and online technical resources including a Google Workspace environment, Microsoft applications (e.g. Word, Excel, and SharePoint), and similar IT resources.
- A valid driver's license, an acceptable driving record, and the ability to meet company vehicle-insurability requirements.
- The ability to pass the required Minnesota DHS background study and complete and maintain TLC orientation, 245D training, and other role-required certifications.
- The ability to read, write, speak, and understand English well enough to write and follow plans, document services accurately, and clearly communicate safety information.

Physical and scheduling expectations
This is primarily a weekday business-hours position. Client needs, meetings, licensing activity, or shared emergency coverage may occasionally require schedule flexibility. With or without reasonable accommodation, the role may involve standing, walking, sitting, bending, pushing, pulling, and physically assisting clients, including lifting up to 50 pounds and frequently carrying up to 25 pounds.
