We are excited to welcome a dedicated and compassionate Physical Therapist (PT) to our growing home health team. In this role, you’ll provide high-quality, individualized rehabilitative care in patients’ homes while working closely with physicians and an interdisciplinary care team to achieve outstanding patient outcomes.

- Perform comprehensive initial evaluations and create personalized Plans of Care
- Deliver physical therapy services in accordance with physician orders and agency standards
- Educate patients and caregivers on safe techniques to improve mobility, strength, and independence
- Reassess patients and update care plans based on progress or changes in condition
- Document patient care accurately and timely, in compliance with state and agency requirements
- Collaborate with physicians, RNs, LVNs, PTAs, and office staff to ensure seamless care
- Participate in interdisciplinary case conferences and quality improvement initiatives
- Supervise and mentor Physical Therapist Assistants (PTAs) as needed
- Promote patient safety, functional independence, and overall quality of life

- Current California Physical Therapist License
- Valid Driver’s License with reliable, insured vehicle
- Ability to travel to patient homes throughout Los Angeles County
- Strong communication, interpersonal, and organizational skills
- Ability to lift, transfer, and assist patients safely
- Experience in home health, hospital, or outpatient rehab settings
- Bilingual skills (any language is a plus!)
- Familiarity with electronic medical records (EMR) systems
- California Physical Therapist License
- Driver’s License & Auto Insurance
- TB Test or Chest X-Ray

- CPR Certification
