About the Role
As an RN Case Manager, you will coordinate and oversee care for an assigned caseload of patients in the home setting. You’ll partner closely with families, caregivers, and an interdisciplinary team to ensure each patient receives thoughtful, individualized care.
This role blends clinical expertise, leadership, and care coordination—giving you the opportunity to make a lasting impact on the lives of the families you serve.
What You’ll Do
- Develop and manage individualized Plans of Care for your patients
- Build strong, trusting relationships with patients and families
- Coordinate care with physicians, therapists, and care team members
- Supervise and support CNAs and IHSS Attendants
- Monitor patient progress and adjust care plans as needed
- Ensure timely, accurate documentation and compliance
- Participate in an on-call rotation to support families when it matters most
- Collaborate with a dedicated and supportive Case Manager team

Qualifications
- Active RN license in good standing (Colorado or compact state)
- Minimum of 2 years of nursing experience
- Home health and/or pediatric experience preferred
- Supervisory experience is a plus
- Bilingual English/Spanish is a plus
- Valid driver’s license and reliable transportation
