Case Manager - OrderID: 5314

We have a client seeking a Temporary Case Manager to assist with support to individuals and families by assessing their needs, developing service plans, coordinating referrals to community resources, monitoring client progress, maintaining confidential records, and advocating for access to housing, healthcare, and social services. 

Hours: Monday - Friday 9am - 5pm

Responsibilities

• Conduct client intakes and assess individual and family needs.

• Develop individualized service plans and establish client goals.

• Connect clients with community resources, housing, healthcare, mental health services, and financial assistance.

• Provide referrals and coordinate services with social workers, healthcare providers, and community agencies.

• Monitor client progress through follow-up calls, meetings, and home visits when required.

• Maintain accurate, confidential case files, progress notes, and required documentation.

• Advocate for clients and help resolve barriers to accessing services.

• Respond to urgent client needs and follow organizational safety and reporting procedures.

• Prepare reports and meet program deadlines and caseload requirements.

• Collaborate with multidisciplinary teams to ensure continuity of care.

Qualifications

• Bachelor's degree is required.

• Experience in case management, social services, healthcare, or community outreach preferred.

• Strong communication, organization, documentation, and problem-solving skills.

• Ability to work with diverse populations and maintain professional boundaries.

• Knowledge of community resources and referral services.

• Familiarity with electronic case management systems is an advantage.

• Must be able to do site visits.
