Hours: M-F, 8am – 5pm
Location: Main: 3401 N Broad St, Philadelphia, PA 19140 with the possible rotation to Jeanes & Episcopal Campus

Duties
The Clinic Navigator RN serves as a central clinical and operational leader within the Temple Faculty Practice Plan, integrating MVP Clinic care coordination with system-wide management of the Actionable Radiology Findings (ARF) program under the clinical oversight of a supervising Advanced Practice Provider. This role is responsible for end-to-end coordination of high-risk patient care and actionable imaging findings across the health system-from identification in the Epic Actionable Findings Manager (AFM) through provider notification, patient outreach, follow-up verification, and documented closure. Functioning as a key liaison between inpatient teams, outpatient providers, specialty services, and community resources, the Navigator supports institutional priorities including reduction of hospital readmissions, regulatory compliance (PA Act 112), and timely completion of recommended follow-up care. The role operates fluidly across clinical and operational workflows, managing a broad scope of findings beyond mandated reporting-including incidental, oncology-related, and lung nodule surveillance-while ensuring care gaps are closed and patients are successfully connected to appropriate, coordinated care.
Education
Bachelor's Degree in Nursing or Combination of relevant education and experience may be considered in lieu of degree Required
Experience
Master's Degree in Nursing Preferred
2 Years Nursing Experience Required
2 years experience in oncology nursing Preferred
2 Years Care Coordination, Navigation, Radiology Follow-up Preferred
2 years working within Epic, particularly with Actionable Findings Manager (AFM) Preferred
2 years lung nodule management guidelines (Fleischner Society) or radiology reporting standards Preferred
2 years complex ambulatory, hospital medicine, or transitional care setting Preferred
Licenses
PA Registered Nurse License or Multi State Compact RN License Required
Certified Case Manager Preferred
OCN - Oncology Cert Nurse Preferred
