Telos Health Systems staffs on a pay per encounter basis Psychotherapists, Psychologists, PsyD or Ph. D, Licensed Clinical Social Workers, LCSW, Licensed Clinical Mental Health Counselors, LCMHC and Licensed Marriage and Family Therapist, LMFT in Skilled Nursing Facilities and Assisted Living Facilities to bring quality mental health services to those communities.
What You Will Do
Perform comprehensive diagnostic assessments of your patients’ needs and psychosocial status.
Develop a comprehensive, individualized treatment plan and goals for your patients.
Additionally, you will provide psychotherapy, group, and family therapy if applicable.
Collaborate with members of the facility’s interdisciplinary team, as appropriate, to develop treatment goals for the patient.
Provide appropriate documentation of all pertinent information obtained in interviews, consultation with other healthcare professionals, referrals, contacts, and supportive services provided to each patient.
This position does not include any case management or discharge planning. Our billing team will handle all billed claims and authorizations. This allows you to focus on solely providing therapy to your patients and your documentation directly into our own Electronic Medical Records.
Complete necessary documentation and reporting in accordance with departmental standards and Federal and State regulations.
Pay Per Encounter role, minimum 2 days a week commitment - out of M-F - you pick the days!
