Telecare Services is seeking a DDA Referral & Community Partnerships Manager to support our growth across the Southern Maryland Regional Office (SMRO) and Eastern Shore Regional Office (ESRO). We are a Maryland-based organization providing high-quality, person-centered support for individuals with developmental disabilities. Our work empowers clients, supports families, and strengthens community partnerships through close collaboration with the Maryland Developmental Disabilities Administration (DDA) and local referral networks
Key Responsibilities
- Drive referral growth for community living group home, supported living, personal supports, and other IDD services through targeted marketing, outreach, and strategic referral development
- Build and maintain strong relationships with DDA case managers, CCS agencies, support brokers, families, and community partners to generate consistent client referrals.
- Market company services through outreach, presentations, transition fairs, provider meetings, and community events across assigned regions.
- Maintain regular communication with referral sources to keep Telecare top-of-mind and ensure a steady flow of qualified referrals
- Serve as the primary point of contact for referral inquiries and guide prospective clients and families through the referral and placement process.
- Collaborate with intake, clinical, and operations teams to match individuals to appropriate programs based on capacity, acuity, and support needs.
- Represent the organization at DDA-sponsored meetings, community forums, and networking events to increase visibility and brand recognition.
- Represent Telecare at DDA-sponsored meetings and forums; Transition fairs and community events
- Track referral activity and provide regular updates on marketing efforts, referral volume, and placement outcomes to leadership.
Required Qualifications
- Prior management and referral experience as a CCS
- 3 years' experience working within the DDA system (Maryland), including familiarity with SMRO and ESRO territories
- Must hold a bachelor’s degree from an accredited institution
- Strong understanding of DDA service models, waiver programs, and person-centered planning
- Prior experience in referral coordination, intake, case management, or community outreach
- Background in Social Work, Human Services, Public Health, Business Development, or related field
- Valid driver’s license and willingness to travel across SMRO/ESRO regions
- Proven ability to engage and collaborate with CCS agencies, Day/Residential providers, and/or DDA staff
Additional Qualifications
- Prior experience as CCS Supervisor (preferred but not required)
- Has performed marketing, outreach, or community engagement activities
Remuneration and Benefits
Salary range: $75,000 - $90,000 (Based on experience level)
Mileage reimbursement
10 days paid time-off and 5 days sick leave
