Employment: Full-Time | Block Schedule
We are seeking a compassionate and dependable Registered Nurse (RN) to join a resident-focused healthcare team in Phoenix, AZ. This is a full-time night-shift position providing hands-on nursing care in a facility-based environment.
Recent RN graduates are encouraged to apply. Candidates with less than one year of skilled nursing experience will be considered, while previous skilled nursing, long-term care, facility-based, or geriatric nursing experience is preferred.
Important Schedule RequirementCandidates must be able and willing to work 12-hour overnight shifts from 6:30 PM–6:30 AM on the facility's established full-time block schedule.

Key ResponsibilitiesAdminister medications and perform treatments as ordered by physicians and within the RN scope of practice.
Assess residents and monitor changes in medical, physical, functional, and psychological condition.
Participate in incoming and outgoing shift reports to ensure continuity of care.
Communicate with residents, families, physicians, nursing staff, and interdisciplinary team members.
Perform initial nursing assessments for newly admitted residents.
Assist with Minimum Data Set (MDS) assessments, quarterly reviews, and individualized care plans.
Maintain accurate clinical and administrative documentation regarding assessments, diagnoses, treatments, and resident progress.
Develop and carry out resident care plans using a team-nursing approach.
Provide limited supervision and direction to supporting staff.
Assist with direct resident care and Activities of Daily Living (ADLs) as needed, including bathing, dressing, eating, toileting, personal hygiene, and mobility.
Monitor medication effectiveness and potential adverse reactions.
Participate in staff development and in-service training.
Follow federal, state, and facility regulations governing professional nursing practice, medication administration, and medical supplies.
Support continuous quality and process-improvement initiatives.
Perform additional nursing responsibilities as assigned.
Active Registered Nurse (RN) license in the State of Arizona
Recent RN graduates are welcome to apply.

Skilled nursing facility (SNF), long-term care (LTC), facility-based, or geriatric nursing experience preferred.
Knowledge of geriatric nursing principles and the aging process.
Understanding of medication administration, pharmacology, assessments, and treatment planning.
Strong clinical documentation and communication skills.
Ability to recognize and respond appropriately to changes in resident condition.
Ability to work effectively with an interdisciplinary healthcare team.
Comfortable providing hands-on resident care and assisting with ADLs when needed.
Ability to provide guidance and limited supervision to supporting staff.
Shift: 6:30 PM–6:30 AM
