Job Summary
The Hospice Aide is a paraprofessional member of the interdisciplinary care team who works under the supervision of a Registered Nurse to perform services necessary to meet patients' personal needs and promote comfort. The Hospice Aide is responsible for observing and reporting patient conditions, documenting care performed, and ensuring quality, continuity, and safety in patient care delivery.
Key Responsibilities
- Patient Care and Support
- Perform personal care needs as assigned, responding promptly to patient requests and reporting to the Case Manager if unable to complete any task.
- Meet patients' safety needs and use equipment safely and properly (e.g., footstools, side rails, oxygen equipment).
- Provide personal care including baths, back rubs, oral hygiene, shampoos, and bed linen changes as assigned.
- Assist patients with dressing, undressing, and feeding as assigned.
- Plan and prepare nutritious meals and perform light shopping when required.
- Measure and record temperature, pulse, respiration, and blood pressure as ordered, following competency verification.
- Monitor and maintain skin integrity, observing for breakdown and reporting abnormalities to the Case Manager.
- Assist in ambulation and exercises per nurse or therapist instructions.
- Provide respite care to families/caregivers when on-site, maintaining a calm and supportive presence.
- Offer and assist with bedpans, urinals, and light laundry as assigned.
- Keep the patient's environment clean, safe, and orderly.
- Collaboration and Communication
- Report significant changes in patient condition to the Case Manager promptly.
- Maintain awareness of caregiver and household dynamics to support positive relationships.
- Participate in quality assessment and performance improvement (QAPI) activities.
- Adhere to organizational documentation and care standards.
- Respect patient and family privacy, personal needs, and environment.
Qualifications and Requirements
- Meets the training requirements established by the state.
- Successfully completed a state-approved Hospice Aide training and competency program, or equivalent licensure.
- Proof of registration on the Home Care Aide Registry through the Department of Social Services, Home Care Services Bureau (preferred).
- Minimum age 18 years.
- Ability to read, follow written instructions, and document care accurately.
- Understanding of hospice philosophy and comfort in providing end-of-life care.
- Satisfactory references from previous employers.
- Ability to work independently with minimal supervision.
- Empathy, patience, and professionalism in caring for terminally ill patients.
- Current CPR certification.
- Licensed driver with an insured and reliable automobile meeting organizational requirements.
