What are the responsibilities and job description for the DODD Shared Living Provider/HPC Independent Provider Portage or Summit Co position at TeamWork by Portage DD?
Are you a DODD Shared Living Provider and/or a DODD HPC independent provider? We have someone looking for both a new home and a new caregiver. Is that you?
- He is a funny, independent, hard-working gentleman when he wants to put his mind to it.
- Looking to stay in Portage County, location is flexible. May consider Summit County as well. He needs to find a new place and a new provider by the end of August.
- He can be very sweet and kind, and has a great sense of humor.
- He will offer assistance to others should he observe someone needing the assistance.
- Mostly independent with hygiene. May need occasional reminders.
- Needs encouragement and attention to maintain appropriate behavior.
- He has been prescribed a PRN for acute periods of anxiety and increased agitation. When working with him, staff need to keep a calm demeanor in their speech and body language. Staff will be attuned to his emotional state and watch for signs and non-verbal cues that he may become upset. Staff will not divide their attention when supporting him (i.e., staff off cell phones, etc.) Staff will avoid power struggles over food, drinks, choice of outings, money, etc.
- He has a IO waiver and does have a behavioral add on.
