Job Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide high-quality, patient-centered nursing care across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. This role involves performing comprehensive patient assessments, administering medications and treatments, and collaborating with multidisciplinary teams to ensure optimal patient outcomes. Responsibilities
- Conduct thorough patient assessments, including vital signs, physical examinations, and health histories using EMR (Electronic Medical Record) systems such as Epic or Cerner.
- Administer medications, injections, IV infusions, and treatments following established protocols while adhering to HIPAA regulations.
- Assist with complex procedures such as catheterization, wound suturing, and sterile processing techniques.
- Monitor patients’ conditions closely in acute care environments like ICU or emergency rooms, managing airway support and ventilator use when necessary.
- Provide specialized care for individuals with chronic conditions such as Alzheimer's disease, dementia, or developmental disabilities in memory or long-term care settings.
- Support discharge planning and coordinate with case management teams to ensure seamless transition of care.
- Maintain accurate medical documentation, including medication administration records and nursing notes.
- Implement infection control practices and adhere to aseptic techniques during all patient interactions.
- Collaborate effectively with physicians, therapists, social workers, and other healthcare professionals to develop personalized care plans.
- Educate patients and their families on health management strategies including nutrition, pain management, and post-acute care instructions.
Requirements
- Valid licensure as a Licensed Practical Nurse (LPN) in the respective state or jurisdiction.
- Proven experience in diverse clinical settings such as hospital medicine, long-term care facilities, hospice care, or outpatient clinics.
- Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth, or eClinicalWorks.
- Knowledge of medical terminology, physiology, ICD coding (ICD-9/10), CPT coding for procedures.
- Experience with specialized areas including ICU nursing, emergency medicine, geriatrics, pediatric care (including neonatal ventilator management), or behavioral health is highly desirable.
- Ability to perform venipuncture, phlebotomy, tube feeding management, and IV insertion competently.
- Strong understanding of HIPAA compliance and patient confidentiality standards.
- Excellent communication skills with the ability to work effectively with patients of all ages and backgrounds.
- Physical stamina to perform patient lifting using Hoyer lifts or similar equipment when necessary.
