About the Role :
You will play a vital role in delivering compassionate and effective care to our patients in post-acute care communities. Your responsibilities include :
- Providing comprehensive primary care services to a manageable patient panel of 15-20 patients per day in a skilled nursing facility
- Collaborating with interdisciplinary teams to optimize patient outcomes
- Ensuring smooth transitions of care with hospice, palliative care, and home care agencies
- Maintaining accurate and timely documentation in our EMR system (Gehrimed)
Qualifications :
Expected compensation is estimated salary range of $69,000 to $96,000 annually with no cap on productivity income potential. This full-time role is benefit eligible.
