Salary Range (Min-Max)
$75,000.00 - $85,000.00

Job Responsibilities
- Contribute to the design and manage the roll out and implementation of population health clinical programs.
- Lead cross functional project teams. Support change management and staff adoption
- Help establish programs goals and alignment with organizational strategy and value-based care objectives. Define scope, timelines, deliverables, and milestones
- Provide insight into workflow, protocols and performance benchmarks. Track risks, dependencies and resource allocation.
- Own and manage the data workflow and reporting.
- Monitor the specific programs KPIs and Provide structured executive updates and dashboards.
- Collaborate with Quality on dashboard reporting as necessary
- Collaborate with IT on all related EMR optimization
- Coordinate with finance and medical leadership on developing pro formas for new programs. Monitor budget impact as deemed necessary.
- Supports grant initiatives related to the provision of the integrated whole person care model and other alternative payment models.
- Provides oversight of the mobile van staff, scheduling and operations. Mobile van operations include participants assistance, emergency preparedness, facilitating clinical documentation improvement, fire drills, and all care coordination activities.
- Ensures all medical appointments are scheduled for participants as indicated for care planning and ordered by the primary care provider and assist as necessary. Ensure that all necessary documents are received and uploaded into the EMR in advance.
- Attends Medical Practice meetings and arranges staff meetings and coverage on a regular basis.
- Respond calmly and effectively to emergencies, such as accidents, medical situations, or mechanical breakdowns, following company protocols.
- Document and report any incidents, accidents, or unusual situations to the appropriate authorities and company management.
- Adhere to established schedules, adjusting routes as necessary to maintain timely service while considering traffic conditions and participant’s needs.
- Only acts within the scope of the individual’s authority to practice.
- Meets a standardized set of competencies for the specific position description established by the PACE organization before working independently.
- Acting member of the IDT.
- Other duties as assigned.

Qualifications
Education: Bachelor’s degree in business administration, Nursing, Health Administration, public health, or related field, or a Medical Practice related Certification from a recognized Medical Practice organization preferred.
Experience
- At least two (2 ) years prior experience in a Medical Practice office-related field, preferred.
- Minimum of one (1) year of experience working with a frail or elderly population or, if the individual has less than one (1) year of experience but meets all other requirements, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
- Experience leading cross-functional initiatives
- Strong data analysis and executive presentation skills
- Master’s degree (MHA, MPH, MBA)
- PMP certification
- Lean/Six Sigma certification
- Value-based care experience
- At least one (1) year of experience working with a frail or elderly population
- Bilingual
- Be legally authorized (for example, currently licensed, registered, or certified if applicable) to practice in the State in which the healthcare professional will perform the function.
- Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.

Individuals must be able to sustain certain physical requirements essential to the job. This includes, but is not limited to:
- Standing – Duration of up to 6 hours a day.
- Sitting/Stationary Positions – Sedentary position in duration of up to 6-8 hours a day for consecutive hours/periods.
- Lifting/Push/Pull – Up to 50 pounds of equipment, baggage, supplies, and ability to lift patients safely and using OSHA guidelines, etc.
- Bending/Squatting – Must be able to safely bend or squat to care for patients, use medical supplies, etc.
- Stairs/Steps/Walking/Climbing – Must be able to safely maneuver stairs, climb up/down, and walk to access work areas Position requires the individual to be able to travel, and walk between sites/locations and work areas throughout the day.
- Agility/Fine Motor Skills - Must demonstrate agility and fine motor skills to operate and activate equipment, devices, instruments, and tools (ie. typing, use of medical supplies, equipment, etc.)
- Sight/Visual Requirements – Must be able to visually assess patients, read orders type/write documentation, etc. with accuracy.
- Audio Hearing and Motor Skills (Language) Requirements – Must be able to listen attentively and document information from patients, community members, providers, etc., and intake information through audio processing with accuracy. In addition, must be able to speak comfortably and clearly with language motor skills for customers to understand the individual.
- Cognitive Ability – Must be able to demonstrate good decision-making, reasonableness, cognitive ability, rational processing, and analysis to satisfy essential functions of the job.
