You will have the chance to use your knowledge and care coordinating social work skills to provide assistance with linking members to insurance, housing, food, employment, personal care, community integration and family reunification. Position requires you to drive members in your personal vehicle to appointments. You will be part of a well-round team and linked directly to the member’s overall treatment plan; to ensure that they are safe, socially thriving, and living a fulfilling life of recovery, in the least restrictive environment.
Requirements:
- Must obtain LLBSW professional license upon hire
- Personal vehicle required to drive members to appointments
