**Job Description**
As an LPN with Team Select Homecare, you'll have the opportunity to work with pediatric and adult patients in the Lansdale area. Your key responsibilities will include:
- Nursing care planning: Develop and implement individualized care plans to ensure the best possible outcomes for our patients
- Preventive and rehabilitative procedures: Initiate procedures that promote health and well-being, and address any potential health risks or concerns
- Patient education: Teach and counsel patients and their families on self-care techniques and implementing care plans
- Medication administration: Administer medications as ordered by medical professionals
