Essential Duties and Responsibilities

  1. Conducts an in-depth, face-to-face psychosocial evaluation to include an assessment of strengths and barriers to becoming self-sufficient.
  2. Determines eligibility based on knowledge of Tarrant County Department of Human Services Policy and Procedures.
  3. Develops a written case plan with the client that includes the goals of self-support, self-sufficiency, and the objectives required by the client to achieve those goals.
  4. Provides vouchers for rent, mortgage, utilities, and hygiene to eligible applicants.
  5. Generates Notice of Denial in the Case Management System (CMS) for the applicant. Provides explanation for denial of services.
  6. Refers clients to appropriate community resources. Provides information and referral to applicants/clients to appropriate community resources such as agencies, churches, community partners, and other public programs.
  7. Provides back up for the Call Center to complete telephone screening intakes.
  8. Conducts telephone screenings and interviews required to serve applicants in the call center.
  9. Monitors client compliance with case plan.
  10. Completes data entry in the CMS for telephone screening intakes and online applications.
  11. Maintains confidential case records and case notes.
  12. Responds to notices of applicant denials, appeals, complaints, and inquiries.
  13. Assists with agency outreach at community events.
  14. Performs all other related duties as assigned.

Minimum Requirements

NOTE: YOU MUST FILL OUT THE WORK HISTORY and EDUCATION SECTIONS OF APPLICATION TO SHOW YOU HAVE YEARS OF EXPERIENCE/EDUCATION AS REQUIRED BY HIRING DEPARTMENT OR BE DISQUALIFIED.

TO APPLY, must have a  Bachelor's degree or higher in Social Work.

TO APPLY, must have Two (2) full-time years of work experience in social work practice.

TO APPLY, must possess a current Texas Social Work license.

MUST PROVIDE LICENSING AGENCY AND LICENSE NUMBER IN "CERTIFICATES AND LICENSING" SECTION OF THE APPLICATION OR RISK DISQUALIFICATION.

Bilingual in English/Spanish preferred, but not required.

Tarrant County will conduct background checks on new hires that will include a criminal background check related to convictions and deferred adjudications in the past Seven years and may include credit reports, motor vehicle records, employment records and educational attainment. A conviction or deferred adjudication is not an automatic bar to employment. Each case is considered individually. 
 

Physical Demands and Work Environment & Other Requirements

While performing the duties of this position, the incumbent is regularly required to sit for prolonged periods of time, stoop, bend, pull and push.  May have to lift heavy boxes or objects up to 100 lbs.
