Job Description
We are looking for a clinically experienced nurse who has nursing documentation/charting skills and is a fast learner with great communication skills.

As an LVN you will:
• Work close to home and as little or as much as your schedule permits!
• Ensure quality and safe delivery of home health nursing services.
• Communicate with RN Case Manager and physician, as appropriate, on a regular basis regarding changes in the patient's condition and/or needs.
• Assist the RN and/or physician in performing specialized procedures and duties.
• Assist the RN in carrying out the plan of care.
• Assist the patient in learning appropriate self-care techniques.
• Perform procedures at a level of care standard within the medical community, the scope of practice consistent with the Vocational Nursing Practice Act and follows the Agency's current policies and procedures.
• Instruct other health team personnel, including home health aides and the patient's family regarding the nursing care needs and other related problems of the patient at home consistent with the LVN Practice Act.
Qualifications:
• Graduate of an accredited nursing program. Current, active license as an LVN by the California Board of Vocational Nurse and Psychiatric Technician Examiners.
• Current CPR, TB, and health clearance and clinical skills self-assessment.
• Minimum of one (1) year LVN nursing experience within the last three years.
