What You'll Do
- Conduct in-person CAS assessments in homes and community-based settings.
- Travel throughout the Franklin region to meet with individuals, families, and caregivers.
- Use a person-centered approach to assess needs, strengths, and support requirements.
- Interview caregivers, service providers, and other stakeholders to gather accurate information.
- Review relevant records and incorporate multiple data sources to support assessment coding.
- Enter data into state-mandated systems and ensure timely, accurate documentation.
- Maintain productivity and quality standards as outlined by the program.
- Coordinate interpreter services as needed for non-English/Spanish-speaking participants.
What We're Looking For:
- Required: Bachelor’s degree in a human services field
- Preferred: Master’s degree in a related field
- Minimum of 1 year of direct experience working with adults with IDD (required)
- Experience conducting assessments and prior CAS experience (preferred)
- Strong analytical, communication, and problem-solving skills
- Ability to work independently and manage deadlines effectively
- Proficient in Microsoft Word, email, and web-based systems
- Bilingual (English/Spanish) strongly preferred
- Valid NYS Driver’s License or NYS ID card
